Please fill out the Medical History Form AND Patient Registration Form, and bring them with you to your first office visit.  New patients may also want to view the Privacy Notice, as the Patient Registration form will require a signature acknowledging your receipt of the HIPPA notice.  These forms are available at the office if you are unable to print them prior to your visit.
