NOTE TO NEW PATIENTS:
Please fill out the Medical History Form AND Patient Registration Form (below), and bring them with you to your first office visit. New patients may also want to view the Privacy Notice (below), which details your rights to privacy, regarding your medical records, as protected legally by HIPAA. The Patient Registration form will require a signature acknowledging that you have received and reviewed the HIPAA notice. These forms are available at the office if you are unable to print them, prior to your visit.
