Insurances and Payment Options 

Please inform us of your current health insurance information when scheduling an appointment.

  

If you have specific questions regarding insurance, or if your insurance is not listed below, please contact us and we will be happy to assist you.

 

For patients whose insurance we do not accept, please check with your insurance companies, as you may be eligible for out-of-network reimbursement.  We also welcome any self-paying patients.

Referrals
Certain insurance plans do not cover annual eye examinations. In this case the patient would need to obtain a referral. This may also be applicable to eye checks, follow-ups, and medical emergencies. If your insurance plan requires a referral from your primary care physician, please contact them prior to your appointment.
