We recommend that you arrive 15 minutes prior to your appointment to allow for check-in and the completion of any necessary forms. If you prefer to fill out the forms before you arrive, we have provided a copy under the Forms tab (above), as a courtesy.

Your most recent insurance card
Your glasses, prescription sunwear, and/or contact lenses, with their corresponding boxes

Please note that, upon your arrival, we will offer you the option to have an Optomap Retinal Exam, which takes a picture of your retina. To learn more, select the Optomap tab, located under the Eye Care tab (above). We also offer the GDx glaucoma screening test, recommended for patients over the age of 40.
