 Preparing for your visit:

We recommend that you arrive 15 minutes prior to your appointment to allow for check-in and the completion of any necessary forms. As a courtesy, we provide our forms online under the FORMS tab, if you prefer to fill them out before you arrive.

 

Also please remember to bring:
  • Your most recent insurance card 
  • Your glasses, sunwear, and/or contact lenses with their corresponding boxes

 

Co-payment and any other fees are expected when services are rendered.

 

 


Please note that upon your arrival we will offer you the option to have an Optomap Retinal Exam, which takes a picture of the retina.

If your appointment needs to be rescheduled, please inform us at least 24 hours in advance, so that we can accommodate other patients.
